All services are self-pay with the prices listed below. Services are provided via telehealth only at this time and clients must reside in Virginia.
Cancellation policy: Two Rivers Psychotherapy and Consulting requires 24 hours' notice to cancel appointments (this 24-hour period starts the day before your appointment, at your appointment time). No-shows and late cancellations (cancellations NOT received within 24 hours or appointment time) will be charged a late cancel fee of $75 (outlined in intake paperwork) at time of scheduled appointment unless the 24-hour cancellation notice has been given. We recognize that changes in schedules come up. However, we reserve your appointment time exclusively for you and ask that you provide a full 24-hour notice so that the appointment time may be opened up for someone else who may be in need of an appointment.
Dr. Chalk believes in maintaining the highest level of confidentiality and client autonomy in choosing a mental health provider and receiving care. In not using health insurance, we have the ability to create a plan for our work together without barriers that are many times imposed by health insurance plans. This also means that your records are kept between you and your provider (meaning that an insurance plan is unable to audit your records and then a non-clinician dictate the mental health services you receive) and we have the freedom to meet as often as you need to.
Upon your request, we will happily provide you an insurance reimbursement form/superbill for you to submit to your insurance provider for possible partial or full reimbursement. It should be noted that superbills provided to you for possible reimbursement must list a mental health diagnosis that will be reviewed by an insurance plan carrier (who will then determine whether or not they wish to reimburse you).
You have the right to receive a “Good Faith Estimate” explaining how much your medical and mental health care will cost.
Under the law, health care providers need to give patients who don’t have insurance or who are not using insurance an estimate of the expected charges for medical services, including psychotherapy services.
You have the right to receive a Good Faith Estimate for the total expected cost of any non-emergency healthcare services, including psychotherapy services.
You can ask your health care provider, and any other provider you choose, for a Good Faith Estimate before you schedule a service.
For questions or more information about your right to a Good Faith Estimate, visit www.cms.gov/nosurprises.
Two Rivers Psychotherapy and Consulting accepts all major credit cards (Visa, Mastercard, and Discover). Payment is due at the time of service. We collect all payments through our bank-encrypted, secure electronic health records (EHR) system which stores the card to be used on file.
If you would like to receive possible partial or full reimbursement from your insurance carrier by submitting a claim directly to them, please request a super bill at the time of your appointment.
